The School of Health Policy &amp; Management will then process the form and make the decision<\/li>\n\n\n\n<li>Students will be notified by mail the outcome of the decision<\/li>\n\n\n\n<li>Students should note the Grade Reappraisal Deadline Dates: Fall Term (F) February 15th, Fall\/Winter (F\/W) June 15th, Winter (W) June 15th, Summer (SU, S1, S2) September 30th<\/li>\n<\/ul>\n<\/div><\/div><\/div>\n\n\n\n<div class=\"wp-block-kadence-pane kt-accordion-pane kt-accordion-pane-5 kt-pane563_25a145-16\"><div class=\"kt-accordion-header-wrap\"><button class=\"kt-blocks-accordion-header kt-acccordion-button-label-show\" type=\"button\"><span class=\"kt-blocks-accordion-title-wrap\"><span class=\"kt-blocks-accordion-title\">Missed Tests\/Exams<\/span><\/span><span class=\"kt-blocks-accordion-icon-trigger\"><\/span><\/button><\/div><div class=\"kt-accordion-panel kt-accordion-panel-hidden\"><div class=\"kt-accordion-panel-inner\">\n<p><strong>Where can I find the Policy on Requesting Make-Up Examinations &amp; Late Assignment Submissions for the School of Health Policy &amp; Management?<\/strong><\/p>\n\n\n\n<p>Please refer to the&nbsp;<strong>Policy on Requesting Late Assignments or Make-Up Exams&nbsp;<\/strong>included&nbsp;<strong>in your course outline.<\/strong><\/p>\n<\/div><\/div><\/div>\n\n\n\n<div class=\"wp-block-kadence-pane kt-accordion-pane kt-accordion-pane-6 kt-pane563_116ba9-3f\"><div class=\"kt-accordion-header-wrap\"><button class=\"kt-blocks-accordion-header kt-acccordion-button-label-show\" type=\"button\"><span class=\"kt-blocks-accordion-title-wrap\"><span class=\"kt-blocks-accordion-title\">Permission to Add a Course (HLST courses)<\/span><\/span><span class=\"kt-blocks-accordion-icon-trigger\"><\/span><\/button><\/div><div class=\"kt-accordion-panel kt-accordion-panel-hidden\"><div class=\"kt-accordion-panel-inner\">\n<p>When trying to register into a course, students may get a message that \"seats may be restricted, please contact the department.\" This is a standard message for the university. If you continue to try and add the course it will allow you to, once sufficient space is available.<\/p>\n\n\n\n<p><strong>How do I seek permission?<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Complete the permission request form on our <a href=\"https:\/\/www.yorku.ca\/health\/shpm\/courses\/permissionrequest\/\" data-type=\"link\" data-id=\"https:\/\/www.yorku.ca\/health\/shpm\/courses\/permissionrequest\/\"><strong>HLST <\/strong>Course Permission Request page<\/a>.<\/li>\n\n\n\n<li>You will receive an email response from the School of Health Policy &amp; Management: either granting permission to enroll or with the reasons why permission was not granted.  Please allow for 2-4 business days to receive a decision.<\/li>\n\n\n\n<li><strong><em>Permission to add a course will not be accepted by telephone, email OR in-person<\/em><\/strong><\/li>\n<\/ul>\n\n\n\n<p><strong>Please keep in mind:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Permission will NOT be granted if prerequisites are not completed.<\/li>\n\n\n\n<li>Course Instructor permission is NOT a guarantee of being granted permission to enroll based on other restrictions that may apply (example: maximum allowable enrolments (course is full) or course prerequisites required).<\/li>\n\n\n\n<li>This is for Health Studies (HLST) Courses ONLY, wi. Once you have completed your form, you would need to submit the form to the <a href=\"https:\/\/www.yorku.ca\/health\/shpm\/contact\/\">SHPM office<\/a> for
